Online Library Card Application Form

In applying for a library card at Albert Wisner Public Library for myself or my child, I agree that:

  • I assume responsibility for the material checked out on this card.  Items checked out on my child’s card are my responsibility until the child is age 18.
  • I understand that material returned late may be subject to overdue fines.  I will pay for all damaged or lost items charged to this card.
  • I am aware of AWPL Loan Periods and Fines.
  • I will not hold the Library responsible for any damage that borrowed items might cause to my equipment.
  • I understand that my child has access to all Library materials and equipment (except for items restricted to adult borrowers for safety reasons or because of the item’s value).  I acknowledge that it is my right and responsibility, not the Library’s, to control what my child reads and watches.
  • I/my child will abide by Library rules, described in the Behavior Policy on the Library’s website.
  • By providing an email address, I understand that I will receive periodic e-mail announcements regarding Library services and programs, and that I may unsubscribe if/when I no longer wish to receive them.

Your card will be prepared and kept at the Circulation Desk.  When you pick it up, please bring with you the necessary identification showing proof of your residency in the Warwick Valley Central School District.  Acceptable forms of I.D. include:

  • A current New York State Driver’s License showing your current street address (not a P.O. Box number) on the front of the card
  • A current Village of Warwick, NY Local Residency card

If you have a photo I.D. that does not show your Warwick street address, then please also provide one of the following:

  • Current car registration or insurance card with Warwick street address
  • Current rental lease agreement with your Warwick street address
  • Recent cable or utility bill with your Warwick street address
  • A recent cancelled piece of first class U.S. mail addressed to you at your Warwick street address

Holds and Overdue notices

For patrons who use email and/or a smartphone, email and text message notices are available for alerting you…:

  • When an item you’ve placed on hold is available for pick-up.
  • When an item you’ve borrowed will be due in 3 days (this is also called a “pre-overdue” notice).
  • When an item is overdue and must be returned or renewed.  Overdue fines may or may not accrue, depending on which library owns the item(s).

Regarding email:  if you provide(d) an email address upon applying for a library card, email is the default notification method.  If you would prefer email notification (instead of “robo-calls”) please provide the Circulation Desk staff with your email address.  Please also alert the Circulation Desk staff if you change your email address, and of any problems you may experience with receiving email notices. 

Regarding text messages:  text message notification is offered as an optional supplementary form of notification.  You will continue to receive a call or email, but a text message to your smartphone would arrive as well.  If you do not have an unlimited plan for texting, this may result in charges.
